On October 10, 2020 at approximately 3 p.m., Bullhead City Police were called to Community Park beach for a report of a child who went under the water and did not surface. Beach Patrol officers began searching in the Colorado River with several others. The 8-year-old boy was pulled from the water and was reportedly in good condition. However, his father was underwater and now missing.

Bullhead City Fire crews recovered Juan Alvarez Ontiveros, 58, from the river and immediately administered CPR. Ontiveros was transported to Western Arizona Regional Medical Center, along with his son. Ontiveros was pronounced deceased at the hospital.
