Public Health Department Vital Records Moving March 31

MOHAVE COUNTY, AZ (March 27, 2023) – The Mohave County Department of Public Health Vital Records program, which provides copies of birth and death certificates, is moving effective Friday, March 31st. The new location will be at the Public Health office at 700 West Beale Street in Kingman. It will be on the west side of the administration building within the Women, Infant, and Children (WIC) office. The hours will be the same as before.  Monday through Friday from 8:30 a.m. to 11:30 a.m. and 1:30 p.m. to 4:30 p.m.

In-person requests for certificates are usually completed while customers wait.  The processing time for mail order requests varies by the type of certificate needed. The office accepts debit/credit card, money order, cashier’s check, or exact cash. Personal checks are not accepted as a form of payment.
